You didn't get banned for making a ban appeal. The reason on the ban list is actually telling you to go make a ban appeal and at the end it says "Ban Evade" but I'm guessing the sentence was too long to fully display in-game.
Here's a quote from ban list:

Yes, he must make his own ban appeal that includes the reason he was banned I just gave you.

I didn't know the ban appeal in the server is forbidden :S For the ban evade-i didn't know there exist thing like this,because this was my 2nd day playing in this server I just get into the server, asked mattz and then he banned me. pls unban the IP :S
